COVID-19 Patient Pre-Screening

We are excited to have the opportunity to welcome you back!

Our office has always utilized infection control procedures that have exceeded all the BC guidelines, however, in light of the COVID-19 Pandemic we have instituted additional guidelines and protocols to ensure your safety. We want to be proactive keeping our patients and team are safe.

Please inform us ahead of time of any changes to your dental insurance plan. To reduce the amount of time you spend in our office, we ask that you arrange in advance to pay your expected portion of the treatment fee. We can take your credit card information over the phone and process it after the appointment, or we can accept an e-transfer to manager@horizondental.clinic

Another way we can maintain physical distancing in the dental clinic is to ask that you fill out any necessary forms in advance. Forms for new patients can be found at Patient’s Resources. Please return them in advance by email to reception@horizondental.clinic or bring them to your appointment.

Regarding children and persons with dental anxiety, or special needs: If you feel that you or your child/family member will not be comfortable or cooperative for a dental appointment given the enhanced PPE worn by the dental team (including gowns, face shields, caps, masks, gloves, etc) and you would prefer to postpone your visit, please let us know. However, we don’t yet know how long these changes will be in effect and may not be able to reschedule right away.

The following Mandatory Screening Questions must be completed, up to 72 hours, before your scheduled appointment. If the patient is accompanied by an “Essential Individual” (parent, support person, or translator), please answer the following for both individuals. If you do not answer or return the questionnaire, your appointment will be cancelled. If you have any questions, or would prefer to do this over the phone, please contact us at 778-765-3928.
